Dangerous incident report

Summary: A haul truck was parked up and shutdown due to rain. The operator remained in the cab. After about an hour, the truck rolled forward 30 to 40 metres before the operator was able to stop it.

Recommendations to industry: Park-up procedures should include requirements for situations where the vehicle is parked up but the operator remains in the cabin.
