Summary: A collision occurred between an underground haul truck and a light vehicle on the mine access ramp. Both operators called on the two-way radio that they were entering the ramp but they both failed to make positive communication with each other.
Recommendations to industry: All mines should review safety bulletin SB18-06 Lack of positive communications. Consideration should be given to the installation of traffic lights at areas where clear line of sight is poor.
