Mining boss dies in freak machine accident

A resources decision-maker suffered fatal consequences after a momentary lapse of judgement.

A mine manager recently passed away from an abrupt machine accident at the Wooten operation, 258km southwest of Little Rock.

The victim tried to perform maintenance work on a Komatsu D65 PX crawler dozer and, when he kneeled on the track, he “accidentally” engaged the lever and put the transmission into reverse.

“The bulldozer track moved the victim to the rear of the bulldozer where he was run over,” the Mine Safety and Health Administration said in a safety alert.

Investigators recommended the following best practices:

  • turn off the engine
  • block equipment against motion
  • operators should train miners on safe maintenance procedures
  • operators should make sure miners are in a safe location in relation to machine parts that can move.
