A resources employee became fatally injured while repairing subterranean equipment.

Authorities recently investigated how a roof bolting machine killed an underground mine worker at an undisclosed operation.

“On 13 November 2023 a rotating drill steel … entangled a miner, causing fatal injuries,” the Mine Safety and Health Administration said in a safety alert.

Investigators found the machinery was still operating during maintenance, and made the following recommendations:

  • turn off or de-energise equipment
  • secure machines against hazardous motion
  • never touch or hold the drill steel, wrench or bolt while rotating
  • do not wear loose-fitting or bulky clothing when working around machinery with rotating parts.
