A heavy vehicle driver became airborne after suffering a deadly collision.

Authorities recently investigated circumstances surrounding a service truck accident that fatally catapulted an operator through the windscreen.

The contracted assistant driller had transported a rod handler at an undisclosed operation on 17 September 2023.

“The service truck left the road, overturned and the driver was ejected. The passenger who was wearing his seat belt was injured, treated and released from the hospital,” the Mine Safety and Health Administration said in a safety alert.

Inspectors discovered the operator used no restraint devices and recommended the following:

  • maintain control of equipment
  • wear seat belts when operating mobile equipment
  • conduct pre-operational inspections and correct any safety defects before operating mobile equipment
  • operate mobile equipment at speeds consistent with the conditions of roadways, tracks, grades, clearance, visibility, curves and traffic.
