Authorities are examining whether a lifting accident involving a heavy vehicle killed a resources employee.
Investigators are trying to determine whether anti-two blocking devices could have prevented a mine worker from dying. A haul truck engine suddenly fell from a crane and fatally struck the victim on 1 October 2022.
“The auxiliary line broke, causing the hook and ball assembly to fall and strike the miner,” the Mine Safety and Health Administration said in a safety alert.
The federally funded agency made the following recommendations:
- ensure cranes have functional anti-two blocking devices to automatically shut off the crane when the rigging on the hoist line gets close to the sheave at the end of the crane boom
- ensure miners stay clear of suspended loads and use taglines when necessary for steadying or guiding suspended loads
- ensure miners conduct thorough pre-operational inspections of all machinery, equipment and tools prior to use.
