A heavy vehicle operator collided with much smaller machinery at an undisclosed open-cut coal operation.
A dump truck driver recently became unable to see a dozer parked behind him next to a windrow, and thought it was a good idea to keep reversing until he heard a bang.
“As the dump truck was reversed, the operator lost sight of the dozer and continued to reverse. The pos 6 wheel contacted the dozer’s ripper box,” the New South Wales Resources Regulator said in an incident summary.
“A truck operator reversed a dump truck into a dozer on a dump.”
Investigators concluded the truck operator should have stopped the vehicle and “reassessed” the situation instead of continuing to reverse.
“They must not proceed [with] placing other workers at risk,” they said in the summary.

i thought they had spotters to stop that type of thing also to stop dozers and trucks going over the edge??
