Serious Incident Alert: Catastrophic engine failure in an underground coal mine

Safety Bulletin - drive shaft fire

While operating in an underground coal mine at Wongawilli Colliery near Wollongong on 4 July 2017, a
catastrophic failure of a diesel engine system occurred. This resulted in internal engine components being
ejected from a cylinder block and being exposed to the mine atmosphere. The temperature of the ejected
components created an ignition source which had the potential to cause an explosion. However, no person
was injured as a result of the incident.
