According to the report, a fuel cart was refuelling a drill rig when a cracked fuel tank sprayed diesel onto the drill rig turbo causing a fire.

The Regulator recommends, “refuelling arrangements for mobile plant should be risk assessed and managed for all mine sites. The potential risk of plant/worker interaction and fire should be controlled. Mines should check the designs of fuel tanks to ensure they are capable of accepting high delivery flows and can handle the pressure of the refuelling system without deformation of the fuel tank including the maintenance of pressure relief valves.”
